InfoLine Referral

  • You can now refer your asthma patients to Asthma Foundation's InfoLine for additional, quality advice and support.

    We provide evidence based and user friendly information and advice to help patients better understand and manage their asthma, and answer any questions that might arise in between GP visits. 

    Your patient will receive a phone call (within two working days of referral) from one of our health professionals who will discuss their asthma control and any issues with them. They will then work with your patient to provide education and support to ensure they have sufficient knowledge and understanding to practice good asthma self-management, and are taking their medications appropriately.

    Patient Consent

    By sending this form to Asthma Foundation NSW, your patient will receive a telephone call within two working days from staff of the Asthma InfoLine to discuss their asthma and asthma self-management. 

    This is a free service.
