Patient Education Referral Service
Health professionals are able to refer their asthma patients to the Asthma Foundation's Patient Education Referral Service.
This telephone service, delivered by 1800 ASTHMA Helpline staff, provides additional education, support and goal setting with patients around asthma self management. The service aims to work with the referrer to achieve improved asthma control in patients.
Who can be referred?
All patients with asthma can be referred to the service, free of charge. The service may be particularly useful for patients who have:
What will be discussed during the telephone call?
The telephone service provides tailored support for the individual and works to reinforce asthma self management as determined by the GP or health care provider.
Key management principles that may be discussed include:
- asthma signs and symptoms and how to recognise worsening asthma
- discussing the role of the prescribed medication in asthma management
- review of delivery device technique
- goal setting with regard to achieving asthma control
- use of validated Asthma Control Test
- offer of a follow up call
How will I know the outcome of the call?
An Asthma Education Summary will be provided to the referrer to outline the discussion that took place and the goals that have been set with the patient.
Even if the patient is not able to be contacted, you will receive notification.
